A Therapist’s Guide to Supporting Mental Health in Your Community

Every September, National Suicide Prevention & Awareness Month invites us to break the silence surrounding suicide, mental health struggles, and emotional pain. But while the campaigns and hashtags are powerful, the truth is: the most impactful prevention starts with real conversations—between friends, family, coworkers, and community members.

Why Suicide Prevention Isn’t Just for Crisis Moments

Often, people think of suicide prevention as something that only applies in emergencies. But prevention is also:

  • Checking in on a friend who seems more withdrawn

  • Validating someone’s emotional pain without jumping to advice

  • Normalizing therapy in group chats and family dinners

  • Reaching out when you’re struggling—before it becomes a crisis

These seemingly small acts can make a lifesaving difference.

Know the Signs: What to Look For

One of the most commonly used tools in mental health is the “IS PATH WARM?” acronym—a simple way to remember key warning signs:

  • Ideation – talking or joking about suicide, writing about death

  • Substance use – increased or risky use of alcohol or drugs

  • Purposelessness – feeling like there’s “no reason to live”

  • Anxiety – agitation, trouble sleeping, constant worry

  • Trapped – expressing a sense of being stuck or hopeless

  • Hopelessness – feeling like things won’t get better

  • Withdrawal – pulling away from friends, family, or routines

  • Anger – increased irritability or rage

  • Recklessness – taking unusual risks, impulsivity

  • Mood changes – noticeable shifts in energy, personality, or emotional tone

You don’t need to be a therapist to recognize something’s off. You just need to care enough to ask.

What to Say: How to Start the Conversation

As therapists, we know that conversations about suicide can feel intimidating. But silence can send the message that someone’s pain is too uncomfortable to hold.

Try these conversation starters:

  • “You’ve seemed a little off lately. Do you want to talk about it?”

  • “You don’t have to go through this alone—can I help you find someone to talk to?”

  • “I’m here to listen, not to fix. What’s going on?”

  • “Are you thinking about hurting yourself?” (It’s okay to be direct.)

Remember: asking someone if they’re suicidal does not put the idea in their head. It shows you care enough to go there with them.
